Open the Student and Volunteer Onboarding Form
Draft: the form is closed while setup and testing are completed. Keep your completed documents. For anything needed now, follow the private submission method your supervisor confirms.
On your first visit, choose I am registering for my placement. Enter your personal or school email, placement details and dates. You can upload any completed items at the same time.
Later, choose I am submitting more documents or confirming a questionnaire. Use the same Google account and personal or school email you used to register. Your reminder emails will include a personalised link.
Upload only the items ready to submit. Leave the others blank and return later. Use readable PDFs or JPG/PNG scans, one file per document, up to 10 MB.
After completing an assigned questionnaire, enter its submission date in this form. No screenshot or certificate is needed for questionnaire confirmation.
Keep copies of your submissions. Do not upload client information or passwords. Blank resource files are for reading and downloading; keep completed forms out of the resource library.
For help with the hub, the form, or an accessible submission option, contact Tristan Yeom. If unavailable, contact Jennie Stone, then Kevin Laforest. Ask your supervisor about placement requirements.
